Anything with a * next to it is required, without it you wont be considered for an unban, anything that doesn't have a * is optional and will just help your case.
*IMPORTANT* - If you're making a ban appeal for chargeback, check your ban message for where to appeal.

*Why do you think you got banned or muted? (The server rules you broke) : I was caught using my hacks.
Have you been banned from Hypixel before? : No this is my first time.
What was the date of your ban roughly? : 18th October.
List one server rule that is different from the cause of your ban : Impersonating staff or youtubers is not Acceptable.
*Why should we unban or unmute you? : I'm really sorry I thought i'd get away with it clearly I didn't.
Any ban appeals saying "My family member/Friend did it" will NOT be considered, it's your account and your responsibility.
If you message a staff member about your appeal by profile or personal message on the forum, your appeal will be automatically denied.
